The West and North London Integrated Care Coordination (ICC) Hub is an ICB-developed and supported initiative delivered in collaboration with the London Ambulance Service (LAS). It brings together senior clinicians from across health and care to make joined-up decisions for people with urgent care needs.
Operating seven days a week, 8am to 8pm, the Hub provides LAS clinicians rapid access to senior clinical advice and alternative pathways, helping patients receive the right care, in the right place, first time. Where safe and appropriate, this can support care closer to home, reduce avoidable conveyance to emergency departments and improve patient flow across the NHS system.
The Hub is already operating in north west London boroughs, and work is under way to expand the model across north central London boroughs. The ambition is to create a single, coordinated ICC Hub supporting all 13 boroughs across west and north London, providing equal access, shared expertise and a consistent service offer.
The WNL ICC Hub would like to hear from GPs and other senior clinicians interested in learning more about the service, helping to shape the next phase or exploring future clinical opportunities within the Hub. Senior clinicians support clinical decision-making, assess referrals, advise LAS colleagues and help identify the most appropriate pathway for each patient.
