Update on frailty coding and LTC LCS communications

BarnetCamdenEnfieldHaringeyIslington

The Primary Care team has developed a frailty diagnosis protocol to help improve frailty coding.

The protocal automatically detects the eF12 score and displays an alert where frailty is indicated. Clinicians can double-click the alert to open the frailty coding template and add the relevent Quality and Outcomes Framework frailty code/Rockwood score. It will also prompt a re-assessment if the eFI2 score indicates worsening frailty.

Read the frailty protocol to find out where the protocal is located in EMIS and view the important update.
