Anticoagulation Service: Barnet Hospital

The Anticoagulation and Thrombosis service is a consultant nurse-led service at Barnet and Chase Farm Hospitals.
Reasons to refer to the anticoagulation service include:
- rapid response clinics
- one-stop specialist assessment anticoagulation service
- initiation of appropriate anticoagulation therapy
- assessment of risk versus benefit and review anticoagulation therapy based on suitability and safety
- bridging plan for procedures
- assessment of adherence to treatment.
The Ambulatory DVT service (rapid response out-patients clinic) to see patients referred from A&E to avoid hospital admission for deep vein thrombosis/ pulmonary embolism.
Eligibility Criteria
Inclusions
- all patients on anticoagulation therapy or who require anticoagulation e.g. warfarin
- patients with thrombosis for extended anticoagulation therapy
- ambulatory deep vein thrombosis / pulmonary embolism where urgent appointment required.
How to Refer
Letter
Referral methods: e-RS, Email
Send referral letter with reason for referral. If blood samples are usually sent for processing to other hospitals, provide up to date blood results (FBC, LFT, renal function). Weight and height is useful.
