if a patient on your list dies outside of hospital.
Exclusions

if a patient on your list dies in hospital.
How to Refer
Referral to the medical examiner will be through SystmOne or EMIS
EMIS form
Complete the NWL ICS Medical Examiner – North West London Community Death Referral Form
